CARD RELATED ISSUES



MEMBER HAS SLADE CARD


The Card is Deactivated

  1. For such cases, it is the card that has an issue and not the member.

  2. Please search for the member using the ADVANCED SEARCH feature.

  3. Use any of the following to search for them:

    • member number

    • member name and scheme name combination.



The Card is Damaged

  1. The member can still be found through the ADVANCED SEARCH feature.

  2. Use any of the following to search for them:

    • member number

    • member name and scheme name combination.



MEMBER HAS A nonSLADE CARD


The Card is a Photocard

  1. Use the ADVANCED SEARCH feature.
  2. Use a combination of member name and scheme name to search for them.
  3. Please enroll the member and let them use the fingerprint process.




The Card is a nonSLADE Card

  1. The ADVANCED SEARCH should be used.
    • DO NOT use the member number on their card
    • Search by combining their member name and scheme name.
  2. Use the nonSLADE process after finding them.




MEMBER DOES NOT HAVE CARD


If Member Forgot Their Card

  1. The ADVANCED SEARCH feature applies for them
  2. Use any of the following to search for them:

    • member number

    • member name and scheme name combination.
